The Social Event Form must be completed for all fraternity/sorority social events, mixers, etc.

Regardless if the event is on- or off- campus, this form must be submitted for it to occur.

**FORM MUST BE SUBMITTED AT LEAST ONE WEEK IN ADVANCE OR CHAPTER MAY FACE CONSEQUENCES.**
