I am responsible for notifying DPS about both the arrival and drop off times of our busses.

I understand that I must submit a list of all attendees (chapter members/dates) to the Greek Life Advisor at least 48 hours prior to my event or the event will not happen.

I have read and understand the Formals Policy, and if I or my chapter do not adhere to the policy, my chapter may be subject to judicial sanctions.

I understand that there is to be no alcohol served nor provided to attendees before, during, or following the event.
